Outreach Specialist/Group Facilitator, Clairton Family Center

Position Summary:  The Outreach Specialist will be responsible to serve as the primary contact between the families at the center and various county and community agencies.  Responsible for intakes, individual assessments, coordination, and monitoring of the delivery of service coordination to high-needs families including case assessments and management, referrals/coordination to social services, medical, and government agencies (etc.) to promote long term self-sufficiency. This position also will identify potential resources to enhance educational, social, and economic opportunities for families. This incumbent will be responsible for bringing more resources to the center that will benefit families that need specialized services. The position also requires the experience and knowledge of the community and key people and agencies in the community. The Group Facilitator will coordinate and facilitate groups at the center, parent educational, ACT Classes, Family Fun Nights, and any other group assigned by the Site Director.

 

Duties and Responsibilities:

Outreach Specialist/Group Facilitator:

1.     Attend community events and meetings

2.     Partner and develop working relationships with community programs and agencies

3.     Provide services to all families of the community regardless of age or family dynamic

4.     Crisis intervention

5.     Coordination of services for families.

6.     Provide resources/referrals for families.  Provide concrete goods to families, food, diapers, formula, etc.

7.     Assist with employment/job readiness.

8.     Housing assistance

9.     Advocate for families, accompany to appointments/court.

10.  Assist with 412 Food Rescue.

11.  Enter all contact/work done with families into Synergy.

12.  Assume other responsibilities as assigned by the site director to assure program operates smoothly and efficiently

13.  Contact Hello Baby families

14.  Deliver Hello Baby Bags.

15.  Follow up with Hello Baby families.

16.  Provide resources/referrals to Hello Baby Families. 

17.  Enter Hello Baby data in Synergy.

18.  Connect Hello Baby Families to home visiting if interested.

19.  Maintain Diaper Bank documentation. Folder with In-take form.

20.  Enter documentation into Diaper bank monthly.

21.  Order diapers monthly.

22.  Pick up diapers monthly.

23.  Coordinate and facilitate groups at the center as assigned by the Site Director. Assist with planning and implementation.  

 

QUALIFICATIONS: 

  1. High School Diploma/GED required with one year’s experience with young children required.  Incumbent demonstrates the ability to work with parents, children, communities and schools.     
  1. Strong communication, interpersonal skills, organizational and recordkeeping skills.
  1. Basic level expertise in current Microsoft Office products.
  1. Excellent organizational skills, initiative, and ability to work with limited supervision .
  1. Ability to prioritize tasks, meet deadlines, and work under pressure.
  1. Ability to comply with AIU policies and procedures.

Position Schedule:  Monday through Friday with an occasional evening and Saturday based on program’s needs.

Salary:$30,250

Benefits:  260 Day FACES Full Time
